A community health center is like any other doctor’s office in that we have licensed physicians, physician assistants and nurse practitioners to provide our patients with the highest quality care, service and preventive healthcare. What is Case Management?

Case Management is a patient management program designed to improve the health of patients with chronic illnesses such as Diabetes, High Blood Pressure, Heart Disease, Asthma, COPD and High Cholesterol. Initially a head-to-toe physical and an in-depth patient chart review are performed, followed by the development of an individual treatment plan. The Case Management staff will explain our preventive health procedures that include services such as vaccines.

The patient benefits and success of this program have been overwhelming. Patients in this program have enjoyed better health, which means increased enjoyment of life.
