What are Health Centers?
Health centers are local clinics that can treat your medical, dental, and other health care needs. They are community-based and led in part by patients at the health center. Health centers focus on meeting the needs of the people and the areas they serve.

What is Case Management?
Case Mangement is a patient management program designed to improve the health of patients with chronic illnesses such as Diabetes, High Blood Pressure, Heart Disease, Asthma, COPD and High Cholesterol. Initially a head to toe physical and an in-depth patient chart review are performed, followed by development of an individual treatment plan. The Case Management staff will explain our preventive health procedures that include services such as vaccines
